ADMISSIONS: Concurrent Enrollment Form - Domestic

If you are a currently enrolled, degree-seeking University of Illinois student interested in taking courses at Parkland College, you must follow the procedures detailed below for Concurrent Enrollment.

  1. Complete a Parkland College application as a non-degree seeking student:
    If you have enrolled in a course previously at Parkland skip to step 2. You do not need to resubmit an application, you only need to submit this completed form and your Academic History.

    You must submit all required documents and be admitted to Parkland prior to registration.

  2. Applying for financial aid?
    Students wishing to apply for financial aid toward Parkland courses MUST also complete the Office of Student Financial Aid Concurrent Enrollment Agreement.

    Note: Parkland’s tuition must be paid by the posted deadline regardless of financial aid disbursement.

    Note: If you are applying for financial aid, the OSFA Concurrent Enrollment Agreement requires an advisor’s/ARO’s/assistant dean’s signature, so you should bring this form with you for your meeting with your college’s designated appropriate point of contact.


  3. Check course prerequisites:
    Prerequisite information is available in the Parkland College Catalog.

    Note: It is ultimately your responsibility to ensure you have met any and all listed prerequisites. Click to verify course equivalences.

  4. Contact college office:
    Contact your academic advisor or appropriate designee* prior to enrolling at Parkland to discuss how the class(es) impact your total credit hour enrollment for the term and how they fit into your degree program.

    Note: It is ultimately your responsibility to ensure the course(s) in which you plan to enroll are applicable towards your current or intended degree program. Verify course equivalences at www.transferology.com

  5. Access Academic History:
    Log in to Student Self-Service to access your Academic History. Parkland College admissions’ office will review your academic history on course prerequisites and to verify you meet eligibility requirements for concurrent enrollment status. You must submit the Academic History as a PDF with this form to register at Parkland. Failure to send the Academic History will result in your Concurrent Enrollment form to not be processed.

  6. Complete this concurrent enrollment form:
    Complete and submit this form along with your Academic History. You may also submit all documentation by fax to 217/353-2640. A confirmation email will be sent to the email provided once your registration is complete.

  7. Tuition:
    All domestic students approved for concurrent enrollment will be charged the in-district Parkland College tuition rate. Tuition must be paid by the posted deadline regardless of payment method.

  8. Requesting Parkland transcripts:
    It is your responsibility to have an official transcript sent from Parkland College to the University of Illinois at Urbana-Champaign within two weeks after completion of the term of enrollment.

Documentation for summer and fall semester is accepted starting April 1 and spring semester is November 1.

This form with your academic history must be submitted together by one of the following methods:

  • Submit online now by clicking the Submit Form button below.
  • Faxed to 217/353-2640
  • In person at Parkland’s Admissions office in U214.
